Mackinac Island, MI--A "my personal health record" train-the-trainer session offered by the Michigan Health Information Management Association (MHIMA) took place Sunday at Mission Point Resort on Mackinac Island.During Sunday's session, volunteer MHIMA presenters learned about the "myPHR" web platform developed by the American Health Information Management Association. The AHIMA "myPHR" web site guides viewers through creating their own personal health record. MHIMA volunteer presenters attending the training session pledged to offer free public "myPHR" educational sessions in their communities throughout the year. |

Tools at "myPHR" include free "adult" and "child" health information forms in PDF and Word formats. The forms make it easy for anyone to keep an electronic record of their health information.Form categories include allergies, identification, medical history, major illness/operations, medications, immunizations, family medical history, and more.For the free forms, visit www.myphr.com, then choose "Start a Personal Health Record," then choose "Free Health Record Forms."
